MISSION STATEMENT
To provide quality outreach health care services and health promotion/disease prevention services to American Indians and Alaska Natives within their communities through the use of well trained CHR's as mandated by Section 107 of P.L. 100-713, dated November 23, 1988.WHAT IS CHR?
A CHR is a para-professional who is trained to provide patient care to the Indian Community of Seminole County.
The CHR Program provides home visits to provide an overall health care management for the patient. Monitor patient wellness and provide transport assistance based on criteria and circumstances. Out of county transports require an WIHC referral.

The priority of services listed are vital to patient care and wellness. The CHR program is a non-emergency program. The CHR program is not a transportation entity.
Why do we require home visits when you request services from the CHR Program?
The CHR will provide a needs assessment in the home of an individual during their visit. CHR's will request personal information from an individual such as: income verification, Medicaid, Medicare, Social Security, Disability, and personal medical history. Personal information obtained from the patient will be an advantage in our advocacy efforts, to provide quality services for our tribal citizens.
